State Labor Organization in Dnipropetrovsk region notes: since entrepreneurs conduct simplified accounting, the personnel documentation for them includes:
- personal cards of employees with the following list of copies of documents: passport, diploma of education with the application, identification code, certificate of availability of privileges (veterans of war, chornobyls, invalids), certificate of compulsory state social insurance, photo (3x4), certificate of marriage, birth certificate of a child (up to 15 years old), medical book (if necessary);
- orders and applications for employment and release, transfer, vacation (including additional, without salary payment), business trips, bonuses, disciplinary penalties etc;
- book of records of orders related to labor relations;
- the journal of the accounting of the movement of labor books and inserts to them (at their own will);
- tables of accounting for the use of working time, graphs of variability;
- documents confirming the calculation and payment of wages;
- contracts of full financial responsibility;
- contracts of a civil nature, acts of performed works;
- fixed-term employment contracts with employees;
- job descriptions;
- labor contracts (including contracts);
- vacation schedules;
- staff schedules;
- rules of internal labor regulations;
- provisions on remuneration (bonuses, etc);
- documents for conducting medical examinations of employees (if necessary).
It should be noted that employers-individual entrepreneurs have the right to create orders in an arbitrary form, but with the indication of mandatory requisites. In addition, they can take as their basis or use official models, approved by the order of the State Statistics Committee dated December 5, 2008 No. 489:
- about the recruitment (No. P-1),
- about giving a vacation (number P-3),
- about dismissal (No. P-4),
- a record of working time (No. P-5),
- calculation information (№ П-6 and № П-7).
An entrepreneur must keep a work record of all employees who work at his main place of work for more than 5 days (in accordance with clause 1.1 of Instruction number 58). Entries in the workbook should be made by the self-employed person upon hiring, in accordance with the employment contract, and at the time of dismissal. All records in the employer's business certify their signature and seal (if they have it).
