Disability letter: what an employee should know

The reason for the appointment of temporary disability assistance may be an issued in the prescribed manner and a properly completed disability sheet.

The Seventh Administrative Court of Appeal provided clarification on the rules for completing a disability sheet, which must be known and checked by an employee before picking up a disability letter from a medical facility.

First, the face of the disabled letter must contain:

  1. The word "primary" or "continuation" is underlined, in the case of continuation the number of the previous disability letter is indicated.
  2. The name and location of the health institution (in full), as evidenced by the stamp and seal of the health institution "For Disability letters".
  3. The disability letter issuing date (the number indicates the number, followed by the name of the month, the number indicates the year).
  4. The surname, the name of the patronymic of the incapacitated person are given in full, the number of full years in figures, gender is emphasized.
  5. Place of work, name and location of company, institution, organization (in full).
  6. Opening and closing of the period of incapacity for work shall be certified by the signature and seal of the doctor.
  7. In the "Get started" column the number and month when you get started is indicated; position, surname of the doctor, which is certified by his signature and the seal of the institution of health care "For Disability letters". In case of temporary disability, "continues to be ill" is emphasized and the new disability letter is indicated.

Note that blots and patches are not allowed in the disability letter. The only exception - in the event of an error in the text is correcting the text, which is confirmed by the entry "Corrected to believe", the doctor's signature and the seal "For disability letters". However, no more than two corrections are allowed on the disability letter form.

Complying with all of these rules will help to ensure that your disability letter would be accepted for temporary disability assistance.
